IV Linzeolid (Zyvoxid)
600 mg x 2 / day	Normal CrCL
אין צורך בהתאמה כלייתית	Renal failure
ביום דיאליזה לתת את אחת המנות אחרי דיאליזה	Hemodialysis

ניטור נדרש: מעקב ספירה (תרומבוציטופניה, לויקופניה), במתן ממושך – סיכון לנוירופתיה, פגיעה בראיה. בשילוב תרופות דוגמת SSRI סיכון ל Serotonin syndrome !

 

	Pediatric dose
Oral, IV: 10 mg/kg/dose every 8 hours, maximum dose: 600 mg/dose	Infants and Children <12 years
Oral, IV: 600 mg every 12 hours	Children ≥12 years and Adolescents
The risk of serious hematologic and neurologic toxicity increases after >2 weeks and >4 weeks of therapy, respectively. When prolonged therapy is required, alternate agents are preferred	Precaution
