IV Piperacillin /tazobactam (Tazocin)
4.5g X 3-4/d	CrCl > 40 mL/min
2.25g X 4/d	CrCl 20-40 mL/min
2.25g X 3/d	CrCl< 20 mL/min
2.25gX3/d +0.75g AD	Hemodialysis

Dosage recommendations are based on the piperacillin component	Pediatric dose
Between 300-400 mg piperacillin/kg/day divided in 3 to 4 doses; Maximum dose 4.5 g, for severe infections max. 4 times daily.	Infants, Children, and Adolescents
100 mg piperacillin/kg/dose infused over 4 hours every 6 to 8 hours; maximum daily dose: 16 g/day (see protocol for extended infusion)	extended infudion dose

Note: piperacillin/tazobactam is a combination product that contains piperacillin sodium and tazobactam sodium in an 8:1 ratio; for every 1,000 mg of piperacillin there is 125 mg of tazobactam
